Tryouts Checklist:

  1. Copy of Legal Birth Certificate 

  2. Medical release 

  3. CEVA membership number as well as proof

  4. Concussion Form

  5. CEVA letter of commitment

All of these will be required before tryouts. Be proactive and come prepared!
